Parent Referral for Mental Health Services


Hello Beachy Community!


My name is Amy Aguilar and I am Beachy’s Psychiatric Social Worker (PSW). I am available for scheduled phone or video appointments for the Beachy Elementary School Community Monday through Friday from 8:00am to 4:30pm.  My primary responsibility is to promote mental health awareness and social emotional development of all students to help them meet their academic and personal goals through a variety of interventions and activities.


PSW Services Offered:

  • Individual short- term/ brief counseling
  • Referrals for long-term counseling services
  • Group counseling
  • Mental health consultation- for staff/ parents
  • Parenting sessions/ workshops
  • Community referrals (housing, food banks, immigration, after school programs, medical services)


Beachy Elementary parents/ caregivers, please complete this form if you or your child is in need of mental health services. Many of our students are processing overwhelming circumstances during this time. It is okay to need help and to ask for it. Click on the photo below to complete the referral form and I will be in touch within two- three business days to discuss your child’s needs and possible services we can provide.

Important info:

Parent/Guardian must provide verbal consent over the phone/video conference in order for me to communicate with students. Parent/Guardian will be asked to verify their name, relationship to student, and address at the beginning of our appointment.


My appointments are not to be used for crisis response. In the event that you need immediate emergency assistance, please call 911.
